Want to save time at the doctor's office? If you download, print and complete the Patient History Form (PDF) before you come in, it will expedite your visit. The most useful information to include in your email is: your name, the nature of your problem (what body part, what you are feeling, how long it's been going on, how it started) and simply list any treatments or tests you've already had (medications, shots, physical therapy, X-rays, MRI, CT scans, etc.)

International patients, please forward your records electronically if at all possible (especially if they are not in English). You can use the Google Language Translator if you want to convert them to English first. Patients from the United Kingdom and European Union in specific, please forward copies of operative reports, as some of you may have implanted devices which are not approved for use in the U.S.
