Complimentary Review of Images

Thank you for inquiring about an appointment with Dr. Jon Hyman. To ensure Dr. Hyman is the most appropriate person to treat your shoulder, elbow, hip or knee pain, we offer a complimentary review of your images for Georgia State residents, and review of images/records for a nominal fee for out-of-state residents, depending upon the volume of records.

Please mail the following pieces of information to our office:

  • Your reports of any X-rays, MRIs and CT scans pertaining to your joint (shoulder, elbow, hip, knee).
  • Any additional records pertaining to your joint (shoulder, elbow, hip, knee) including physical therapy notes, operative reports, office notes or injection reports.

*PLEASE BE SURE TO SEND ALL OF THE INFORMATION TOGETHER. YOUR INFORMATION WILL NOT BE REVIEWED IF THEY ARE RECEIVED SEPARATELY.

Please include a self-addressed stamped mailer, so that we can return your films to you. We will not return print material.
